Sometimes HLDJ likes to hum at me

Which, is nice, except it's a dull bass droning and I can here my keystrokes while typing (vaguely). It's like there's no file input and instead it just sort of freaks out by trying to get my mic input.
1) Does HLDJ launch the game correctly for you? Yes.
2) Does "HLDJ Loaded Successfully" appear in console on starting the game (with no errors)? Yes.
3) What is the console output after selecting a song, or issuing other HLDJ commands such as "hldj_next"?
After song select I see:
Host_WriteConfiguration: Wrote cfg/hldj_cmd_h.cfg
Loaded : my_song
4) After loading a song, perform a search in your Steam directory (without stopping HLDJ) for the file "voice_input.wav" - does it show up? where? Yes, in [steam]/steamapss/[my_account]/team fortress 2/
5) Does voice normally work for you? If not, or you do not have a mic, have you remembered to set up the appropriate options in your OS and the game? Yes.

Normally, it works fine. If I create my own server, it works fine. However sometimes I'll get the issue described above and it won't go away. I fear it may be server specific.
The game is TF2, the OS is Win7 32bit, I have a mic and everything works fine and is stable and normally performs fine except for this issue.
EDIT: I've spotted the thread discussing the interference from the cvar "sv_allow_voice_from_file". I tested this on a server where this was set to 1 and I still had the issue. Interestingly, this doesn't seem to have any bearing on which sound file I attempt to use. In the last test, I went into a server that allowed my to use voice from file, got the hum. Then I changed track and it worked fine. The weird thing is, the sound files are known to be good as they've worked before. Also, ALL sound files have been converted to single channel ~11kHz wav files so I'm 99% sure the fault isn't with the sound files.
