HLDJ works, then doesn't work again

1) Does HLDJ launch the game correctly for you? Yes it does
2) Does "HLDJ Loaded Successfully" appear in console on starting the game (with no errors)? No, it says "some files could not be written (1 error)"
3) What is the console output after selecting a song, or issuing other HLDJ commands such as "hldj_next"? The correct output "Loaded: Busta Rhymes - Arab Money

4-a) After loading one of the sample audio files, perform a search of your Steam directory (without stopping HLDJ) for the file "voice_input.wav" - does it show up, and if so, where? I could not find that file in my steam dirrectory
4-b) If it exists, open it in an audio player: does it sound like the audio file you loaded? N/A
4-c) If it exists, return to the game, and enter the following into console:

Does the audio file start to play? No, it only records from my mic and plays it out of my speakers
5) Does voice normally work for you? If not, or you do not have a mic, have you remembered to set up any applicable options in your operating system and/or the game? Yes it always has

A bit of extra information, The first time I used HLDJ it worked just fine until the second time I used it and I was given this same problem. Then, I tried using those commands and they did work just ONCE. After I exited TF2 and HLDJ and opened them back up again, they failed to work again and I was given this same problem. Also, it's the same case for all my other games.

First try disabling Steam's Cloud back-up service for your game. Then search for and delete from anywhere in your Steam directory any files containing "hldj" in the filename.
Finally, if you still cannot find voice_input.wav, post a screenshot of the contents of the following directories (after you have loaded an audio file):
- your game directory
- the 'cfg' folder in your game directory
- the directory one level up from your game directory

After doing all that and erasing all the HLDJ files from my steam directory, I see the voice_input.wav file and it does sound like the song I'm using, except when I press my play key in game, it still only records from my mic and then it comes out of my speakers.

For most Source games, such as Team Fortress 2 and Counter-Strike: Source, you will need to test on a server that allows playing of audio files, in console: "sv_allow_voice_from_file 1" and ""sv_use_steam_voice 0".
